To make an appointment and claim Medicare benefits, you will need a referral from a medical practitioner. If you do not wish to claim a Medicare benefit or do not have Medicare, an appointment can be booked without a doctor’s referral. The consultation fee is the same.
To build a comprehensive medical profile for review by the doctor prior to your appointment we ask if you could please complete our online appointment and patient questionnaire. Please also attach a copy of your referral and any relevant reports including hearing tests, audiology reports and radiology scans.
New patient referrals are triaged by Professor Atlas and our practice nurse to ensure that all required assessments are organised before your consultation. You will be contacted by our staff or an appointment letter will be emailed or posted to you.
If you are unable to complete the online referral and questionnaire, please email your referral and reports to [email protected].
